Upon acceptance and approval of this application;
Upon the basis of the Constitution, Bylaws, Agreements and Regulations of the Association now in force,
I/We will confirm to and agree to be governed by the same or to any alterations which may be made by proper
authority during such Membership.
The Association emblem will only be displayed on Company premises and equipment so long as the Company remains in good standing. Payment of dues will be in accordance with rate set by the Association.

Plus monthly dues of 10 cents per hour worked per trades person (journeyman & apprentice) for work performed under the jurisdiction of plumber, gasfitter, steamfitter-pipefitter and welder for residential, commercial and institutional and 3 cents per hour worked in the industrial field to a yearly maximum of $7500.00 per company (including labour sub-contractor hours).

MEMBERSHIP DEPOSIT OF THE FOLLOWING AMOUNT WILL BE SENT: $500.00 + $25.00 = $525.00.
