GROUP INSURANCE BENEFITS and MONTHLY PREMIUMS

The following is a very brief summary of the benefits included in the MCA benefit plan that is underwritten by Sun Life Financial.
- $40,000 Life Insurance
- $40,000 Accidental Death and Dismemberment
- Dependent Life – $10,000 spouse; $5,000 each child
- Dental 80% basic – 50% major and children orthodontics
- Extended Health Care – 100% prescriptions, semi private hospital, ambulance and other covered services; plus emergency Out of Country medical expenses.
The monthly premiums for above benefits are :- : $277.91
- : $125.48
- Long Term Disability – 66.7% of covered monthly earnings to a maximum $5,000.
$2.02 for each $100 of Long Term Disability benefit.
Optional Life insurance coverage is available up to $250,000 upon request and acceptance by the insurance company.
